Our Team

VAAACares® executive team members are nationally recognized leaders in the development of aging programs and services, approaches to addressing social determinants of health, and reduction of acute care readmissions. We were recognized for outstanding business innovation by The Robert Wood Johnson Foundation, received a 2022 USAging Aging Achievement Award for our work to develop person-centered care coordination accredited by the National Committee for Quality Assurance (NCQA), and were named Health Quality Innovator of the Year 2021. Our nationally recognized Care Transitions program has coached over 45,000 Medicare, Medicaid, and underinsured patients recently discharged from hospitals, thereby avoiding 7,800 expected readmissions and effectively contributing over $62 million in healthcare savings while improving patient satisfaction. Included in our array of services for managed care and Medicare Advantage are health screenings and assessments to assist with vital community resource referrals and ensure members are appropriately stratified. We are currently completing thousands of health screenings and assessments weekly for multiple managed care organizations.
